CHC or direct payments

Hi, could you please advise whether CHC professionals have a role in decision-making for a 19-year-old with PMLD? Do they have any involvement in resolving disputes between medical teams and parents regarding decisions such as surgery or medication administration?

We’re unsure whether to apply for CHC or remain with social services and direct payments. Any advice would be greatly appreciated.

Hi I believe so but C.H.C. starts with your S/W who has the appropriate form to fill in which is on a 12 question point system of which you have enough points to qualify for application to receive C.H.C. This is the format we went through........
