Amending existing Deputyship order

My ex has a deputyship order for our son, who has limited capacity. I'm excluded from helping him make any financial decisions. I'd like the current order to be updated with my name added. Two questions:* Which of the CoP application forms do I need to submit?

* At what point do I need to notify the people I have listed on the form (incl my ex)?
