probable ASD diagnosis back in 2011.apparently was formalised , no idea only just been given a piece of paper ,was from primary school referral ,2012. I’m not c.c in it.Two wks ago after asking new Gp, needed formalising . This shows his address is wrong, GP wrong practice,EHCP states, global learning difficulties since 2016 yet no ones assessed him, landed with LD team after not wanting to live thoughts so LD nurse thought , it was acceptable to. do as they pleased, one particular person thought they was takin him for bloods despite work being done to support him, only saw him once ,even put wrong name for me on patient passport. Mess to avoid.
