I’d it possible to have lupus nephritis with a normal creatinine ?

Very unlikely unless you’re older (>70 yo) with no muscle mass. A normal creatinine would actually be high in these folks since creatinine comes from muscle. If you don’t have any muscle then your creatinine should be quite low.

It’s also more important to focus on protein in the urine because this is how lupus nephritis is typically monitored and picked up. There is one class of nephritis that is slow progressing and doesn’t cause tons of proteinuria. It can cause chronic kidney disease. However, these people rarely develop end stage kidney disease, which requires dialysis or transplant.

Thank you so much this is a great help. I seem to have all signs on dip urinalysis however my u&e are ok . I think this means I’m not in the danger zone .

They should monitor it and trend labs to make sure it doesn’t progress. I’m sure they’re on top of it (I hope). I’m the opposite. My creatinine is high and eGFR is low but no protein in my urine.

Also, men tend to have more severe disease. We don’t know why. Maybe they tend to seek care later in their course.

I do hope you figure out what’s going on. If you’re unexpectedly swelling (esp legs) or short of breath, make sure to let someone know. This would indicate high proteinuria.

I will . I do get swelling in my fingers and feet . Usually late in the day I put it down to the end of the day. Thankfully no shortness of breath
