You can request your notes via the particular hospital’s website ( often). Otherwise maybe contact the ICU to find out how you retrieve your records
If you were in a UK hospital, you can phone the PALS (Patient Advice and Liaison Service) for your hospital.They will have the details of the relevant person who you need to contact at the hospital trust.
That is what I did and got a voluminous ICU report sent to me (which I still haven't had the courage to look at yet).
The easiest way is to go to the hospital website. Freedom of information. They will copy everything including X-rays etc. it’s free. They will give you an expected time of delivery but it may take more than 4 weeks. It may be possibly to ask for a PDF copy rather than a printed copy mine was nearly 5 reams of paper together with a cd for the X-rays.In the end I scanned it all and created my own pdf (I had a printer that would scan both sides on a feed). The pdf means that you can search it. Everything was typed up except for the physio notes.
I requested my notes from the admitting hospital (I was transferred to another hospital for icu) and they were short and not typed.
It took me a while to read the icu notes and a I did few days at a time. It explains a lot and in particular the care I had there.
Here in this part of Wales you get an ICU Diary which the nurses fill in every day. This is is given to you a few weeks after discharge.
Sadly not in every hospital. I was in ICU in Cardiff Heath hospital in sept 2020 and they didn’t do a daily diary. I’ve really struggled with this as others in my support group who were in different Welsh hospitals, did get one
Hi. Your hospital may not have kept a diary. I was not offered one, but as I have written I obtained my hospital record, which included the daily clinical notes, test results, letters and scan results. Its as good as a diary.....if you can read the writing, as most is hand written.
Thanks Marty, I managed to get those but just for Cardiff. I need to get my notes from Newport and Abergavenny as I was across three hospitals. Main one was Cardiff though but I had emergency surgery in Newport ahead of Cardiff ICU. I need to print all my notes and go through them - I did notice a lot are handwritten
I was lucky in that Bucks NHS Trust covered all four hospitals in the area.
Hi. I obtained my full hospital records, and initially contacted the PALS of my local NHS Trust, and they refered me to the 'access to your health records dept' for the local NHS trust. I received copies of all my clinical notes, test results, letters, and on two discs my xrays and scans. It took less than two weeks to receive these. It was very interesting reading as I could not remember much after arriving at A&E and waking up in ICU after Sepsis, and again when in ICU with covid. My trust is Bucks.
