Hi James H

A lot of people would like that table.

However, everyone metabolises Warfarin at different rates and also food and drink affects it too, so a table is not a possibility.

Best wishes.

James, for example, despite eating a consistent diet, my Coumadin doses have ranged from 7mg to 17mg daily over the past 7 years. I'm currently on 11. My INR range is 2.5-3.5.

and to prove the point, my usual dose is 4 and 5 alternate days and my target is 3.9

INR results vary depending on the individual and some factors are not well understood such as the role of food and drink, other medications, the amount of antibodies in the blood, etc.

This is why each person on warfarin needs individual tailored dosing based on regular tests.

Some people on warfarin find that after a while their results settle down and the tests do not need to be done as often, but some people never settle down and continue to need frequent tests.

As INR is affected by many factors which can change over time, such as if you eat less or get a cold, you will always need the tests while on warfarin, even if they are not as frequently needed.
