Discharge plan - help making sure we get what we need

Any advice on discharve plan? How do I ensure we get what we need?

Ask as many questions as you can. Do as much research as you can. Call the helpline. Use this forum. Check all basically!

The only aftercare I received was a contact number for the Specialist Nurse on the Neurology Team and a recall appointment. To be fair, I declined the offer of visits from a social worker.

I hope all works out well for your dad after his discharge ; for further support you can always phone the Headway helpline on 0808 800 2244.

Hi

Not sure what you might need - and I don't think I came out with any plan whatsoever - so had to rely of friends and family chasing things on my behalf.

Community neuro rehab team were great - didn't hear about them until a few weeks after - worth contacting asap.

Headway - maybe trying to see if they will visit at home ?

Remploy - very good at talking to me about money/employment issues and great at talking to me about benefits ( I wasn't eligible for anything apart from taxi to work later on ) . And very well informed about medical issues.

Own Gp practice - make sure they are on board about all of the issues faced - I even had deal with my receptionists that I would walk to see them every day - even if just to say hello ( I live alone so important that someone knew if I was absent).

These are my initial thoughts for you.

One thing I would advise is......

If you are going to take any medication please make sure you have a list of what tablet and what time you are ment to take it.... I struggled with this!

I wasn't well enough to be in charge of my own medication. Had to have carers in to ensure that I took them at the right times. Worth thinking ahead as to what may be needed.
