Gluten Free diet?

What do people mean when they refer to the "Gluten Free diet"

a, that you have to eat the 200ppm of gluten in the diet?

b, that you avoid gluten as much as possible?

c, total gluten avoidance?

I avoid everything even stuff 'made in a factory that also handles wheat'. I react even when I dont eat food with gluten in, just being in a normal gluten-laden environment gives me symptoms even when I am very careful.
