The headline is: If you have a psa over 1,(under 50y/o) or 1.5 (over 50y/o) demand routine psa tests to watch, despite the “standard clinical normal range” of 0-3.9. Something is wrong and it’s not ok to go home and think everything is ok.
I had 3 psa test done (as response to blood in my urine ) under the age of 45. All 3 test came back high but in “normal” range. My dr. sent me home saying your fine, call me if it happens again.
Well I was clearly not fine and at age 48 was de-novo (stage 4,Gleason 9) with incurable disease.

Exactly right... It was why I made sure my brother's course of treatment was correct and went smoothly... I had already been screwed in my experience.... I got infected after having my TRUS and being given an antibiotic that was no longer recommended to be given for the procedure due to E coli resistance.... To this day, I wonder how much cancer spreading occurred.
My brother got the MRI guided transperineal biopsy... no infection.... had a great surgeon for his RP.... so far so good.... 1 year out ...fingers crossed...
The point is ....how many people do not get the testing or follow up needed by the medical community... My urologist told me afterwards, "We'll just put you on lupron and everything will be fine..." Left him in the dust and have been at Cleveland Clinic ever since...
This was almost exactly my experience as well. PSA 0.8 in 2018, 3.7 at my 2020 physical and 3.5 at my August 2021, no alarm bells because "normal range". Went metastatic with a PSA of 63 in August 2022.
In hindsight this was completely idiotic as a 400%+ jump in any marker in medicine should raise eyebrows, even within the normal range.
As counterproductive as it is to relitigate the past, I’ll most certainly do my part to make sure no one I can reach falls victim to this miscalibration in the healthcare system. I expect As more and more men get Pc earlier and more aggressive, they are going to be forced to re-examine their guidelines. God willing.
Men should be taught to plot their PSA from a young age. Its easy and cheap. By not advising men to do this, the industry produces many good customers.
