I THINK YOU SHOULD STRAIGHT AWAY GO TO SHORT ACTING INSULINS TWICE OR THRICE IN A DAY AS PER REQUIREMENT,OR U CAN TAKE NEWER GLIPTINES LIKE LINAGLIPTINE ONCE IN A DAY WITHOUT MUCH HYPOGLYCEMIA

with renal failure patient i would suggest insulin.though there are certain report if the renal insufficiency is not progressive one can switch over to metformin but concern your doctor
