The pathology
Parkinson's disease represents the most common neurodegeneration among movement disorders . The causes are not known: environmental ones are suspected and a "familiarity" is often found . It frequently occurs around the age of 60, but it is not a disease of the elderly and increasingly affects young people. About 10% are people under 50 . The best known of the symptoms is tremor , but so are slowness of movements, muscular rigidity, a certain inexpressiveness of the face, and difficulty in writing. Some symptoms such as reduced sense of smell, sleep disturbances, depression and constipation (tiredness) can precede the onset of movement disorders by years. But it is difficult to relate them to Parkinson's.
Today "there is substantial evidence on the role of neuroinflammation in the mechanism that leads to the death of nerve cells at the level of the dopaminergic system. The underlying mechanism is determined by an increased vulnerability of dopaminergic neurons to various neurotoxic insults, which can determine neuroinflammation. In this context, numerous scientific data underline the role of neuroinflammatory phenomena in the uncontrolled progression of pathologies such as Parkinson's" explains S tefania Brotini , neurologist at the San Giuseppe hospital in Empoli.
The treatment
There is currently no known cure for Parkinson's . However, various pharmacological therapies are available which, especially in the early stages, guarantee good control of the symptoms: there are many drugs available, and the choice of the best combination is made by the specialist neurologist, taking into account the patient's characteristics.
Scientific research
" Research on Parkinson's disease in recent years has been dedicated to the study of neuronal pathology , but also of all other non-neuronal cells. Among these in particular microglia and astrocytes, which represent the main protagonists in the neuroinflammatory process. What what emerged is that, contrary to what we believed in the past, the neuroinflammatory process arises before the degeneration of neuronal synapses, which means that it is not the consequence as we believed in the past. Therefore the degeneration of Parkinson's is secondary to chronic neuroinflammation and not vice versa ", explains the expert. This discovery led to the study of lipid molecules that may play an essential primary role in combating chronic neuroinflammation. "These include ultramicronized Palmitoylethanolamide (PEA-um), which is able to counteract harmful phenomena affecting the central nervous system. Endogenous PEA counteracts the onset of neuroinflammatory phenomena through the inhibitory control of non-neuronal cells, when these are hyperactive", she explains.
"We studied the exogenous administration of PEA-um in a group of parkinsonian patients. The results were exciting: the dyskinesias were significantly reduced and there was also an increase in the duration of the dopamine effect - says Brotini - In addition in fact, Pea-um can be an effective adjuvant to classic therapy to reduce involuntary movements or the duration of motor block. It therefore reduces the side effects of the disease and of traditional therapy and delays the course of the disease even in the advanced stage In summary, the addition of ultra-micronized PEA demonstrated a notable improvement in motor symptoms, but also in many non-motor symptoms."

The successful studies used um (ultra-micronized), not micronized. I use micronized as it is easier to manage, but I also have um and should really be using that.

I feel Melatonin is a better option than PEA.
I'm kind of partial to natural healthful things that are native to the body and melatonin meets that criteria. Melatonin also has very significant research supporting its use for many health issues and PD is one of those.

So I was bouncing around and found this article: Palmitoylethanolamide/Luteolin as Adjuvant Therapy to Improve an Unusual Case of Camptocormia in a Patient with Parkinson’s Disease: A Case Report 2021 ncbi.nlm.nih.gov/labs/pmc/a... This is a single case report about a fairly unusual condition related to PD and did not hold my attention.
But then I stumbled upon: Randomized clinical trial "olfactory dysfunction after COVID-19: olfactory rehabilitation therapy vs. intervention treatment with Palmitoylethanolamide and Luteolin" 2021 pubmed.ncbi.nlm.nih.gov/341... and this showed the combination of Palmitoylethanolamide and Luteolin improved the sense of smell in people who had COVID, and it worked better the longer their sense of smell had been diminished.
So... It treats Camptocormia in a patient with PD AND rehabilitates the olfactory system. Let's search on "Palmitoylethanolamide" + "Parkinson's":

" Background: Parkinson's disease (PD) is the subject of intense efforts to develop strategies that slow down or stop disease progression and disability. Substantial evidence points to a prominent role for neuroinflammation in the underlying dopaminergic cell death. Ultramicronized palmitoylethanolamide (um-PEA) is well-known for its ability to promote the resolution of neuroinflammation and exert neuroprotection. This study was designed to assess the efficacy of um-PEA as adjuvant therapy in patients with advanced PD.
Method: Thirty PD patients receiving levodopa were included in the study. The revised- Movement Disorder Society/Unified Parkinson's Disease Rating Scale (MDS-UPDRS) questionnaire was used to assess motor and non-motor symptoms. Clinical assessments were carried out before and after addition of um-PEA (600 mg). MDS-UPDRS questionnaire total score for parts I, II, III, and IV was analyzed using the Generalized Linear Mixed Model, followed by the Wilcoxon signed-rank test to evaluate the difference of each item's mean score between baseline and end of um-PEA treatment.
Results: Addition of um-PEA to PD patients receiving levodopa therapy elicited a significant and progressive reduction in the total MDS-UPDRS score (parts I, II, III and IV). For each item, the mean score difference between baseline and end of um-PEA treatment showed a significant reduction in most nonmotor and motor symptoms. The number of patients with symptoms at basal was reduced after one year of um-PEA treatment. None of the participants reported side effects attributable to the addition of um-PEA.
Conclusion: um-PEA slowed down disease progression and disability in PD patients, suggesting that um-PEA may be an efficacious adjuvant therapy for PD. "
