Possible Criticisms of the UPDRS

I've been using the Unified Parkinson's Disease Rating Scale (UPDRS) to determine whether I've reached the optimal level of B1, per the Constantini Protocol. I've been considering two possible questions/criticisms of it:

1) Are the rating of each type of impairment weighted in proportion to their importance? I can't tell. For instance, #13 (Falling) may be rated from 0-4, as may be #28 (Posture), but what if Falling is 5 times more important than Posture? Then wouldn't the correct rating scale be 0-20 for Falling and 0-4 for Posture?

2) Many Parkinsonians suffer from constipation and/or incontinence/frequent urination. Why aren't these covered?
