Voluntary Survey for Parkinson's Disease

My name is Sam and I am a science research student in High School who is asking for your voluntary participation in my research project. I am conducting a survey to answer my research question of a correlation with Parkinson's Disease and REM Sleep Behavioral Disorder. This survey contains questions about your experience with Parkinson's Disease and possibly REM Sleep Behavioral Disorder(RBD). If you participate, you will be asked to complete a questionnaire that should take approximately 5-10 minutes to complete and no personal information will be collected.
