We have published a new survey which has been created by someone living with Parkinson's who would like to understand more about products dispensed to you by your chemist and how you feel about whole experience.

We would be really grateful if you would take part; the survey should take no longer than 5-10 minutes to complete and is intended for individuals with Parkinson's only.

I filled out the form. It is a marketing study to help the "...individual living with Parkinson's" create new packaging. I suggest spending 5-10 min elsewhere.

CPT- Helen, i started this but stopped as I dont live in the UK and I think my input would skew the results.
