Survey to understand which Parkinson's symptoms impact your quality of life

We would be so grateful if you and your spouse/partner/family members could spare 20 minutes to complete a survey.

The purpose of this survey is to collect important information that will provide a more complete understanding of the symptoms you experience as a person with Parkinson's (PwP), how they may change through the day and how they impact your life. To help us gain a complete picture of living with Parkinson’s we would also like to survey the person who you view as a key support to you for example, your partner or a key family member or a carer. I accidentally hit a key - I think it was the delete key resulting in complete loss of all answers. It took me ages to fill in this survey certainly more than 20 or 25 minutes. I am not going to bother going through all that again. Also please would it be possible to keep the headings visible while scrolling through the options / multiple guess etc

A very detailed and thought provoking survey. A good regular process to undergo, regularly. Pete is correct about scrolling, but nothing serious.
