It is important for anyone taking carbidopa levodopa to supplement B6, but it must be taken at a different time. See here for details:

The article mentions that vitamin B6 deficiency is rare, but that is probably not true for many of us! The information on the interaction between carbidopa and B6 as PB describes here above shows that for PwP with C/L medication that risk is significant. Your doctor and certainly your neurologist should inform you about this. From the responses to earlier post it appears that this is rarely the case. Even worse, probably they are not even aware of this relationship and the possible consequences.

In addition, several studies indicate that between 30% and 45% of people with PD suffer from some form of neuropathy, which could possibly be caused by too low a B6 level.... But also due to too high a B6 level (which has been warned about here before) as a result of consuming too much B6 which is often part of vitamin B complexes. Your doctors should warn you about too.

As an experience expert, I know that the consequences can be significant. The negative spiral of poorly functioning C/L medication in increasingly higher doses and as a result the almost complete breakdown of B6, resulting in an irreversible neuropathy. This became clear by having my B values tested myself. After a few days with an extra vitamin B6 (P5P) supplement, my situation had already improved greatly. Now after almost a month I have even reduced my medication by 70% and I am at the level of the starting medication from almost 2 years ago! Many of the typical PD complaints have reduced or even disappeared. Let's hope the "awakenings" continue… Unfortunately, this probably is not the case with the neuropathy complaints, although they seem to have stabilized.

It is still difficult to find the optimal dose of extra B6 and the right balance with C/L medication. This is all do-it-yourself now, because I can't find anyone who can say anything meaningful about it. It is suggested that it is best to match the mg of B6 to the daily mg of carbidopa intake. A supplement with P-5-P is more absorbable than pyridoxine and is probably also safer in higher doses. In addition, I keep at least 2 hours between the B6 and C/L intake. Only a new blood test can tell if I guessed correctly. I will keep you informed about this at HU.

So once again the urgent advice to everyone:

GET YOUR B6 TESTED TODAY!!!!

Totally agree. A few years ago I published a brochure on vitamin B6 and levodopa with carbidopa. I think it can be useful.

Does it make sense to currently restrict vitamin B6 so severely since taking Sinemet (Levodopa with Carbidopa), Madopar (Levodopa with Benserazide), etc?

Our answer is categorical: no sense at all (within the scientific and reasonable limits set by government recommendations, information in the package leaflets of levodopa manufacturers and publications of prestigious neurologists: 1.3 to 25 mg/day in adults, unless specifically indicated by the doctor: age, diseases, medications, etc.).

Today, this is a prejudice, inherited from the years when levodopa was administered alone and which has survived for too long against the opinion and the writings of numerous neurologists and against what the manufacturers of Parkinson's drugs report.
