An app to track symptoms to make consultations more effective. Yesterday I made 2 pages of notes in Word regarding my condition and medication because the neuro appointment I thought was scheduled for Thursday was in fact due a month later (early onset dementia?) I realised I would probably forget all the issues I wanted to discuss. Maybe this app is a good idea

Dr. Laurie Mischley has a survey that you can take regularly to track symptoms. At appointments doctors can only test motor symptoms. This survey asks mostly about non motor symptoms. You can show your doc the scores and progression for a more complete evaluation of your condition.
