I now have extreme fatigue most afternoons starting about 3:00 until 5 or 6. Any ideas. This started fairly recently.

I know many Parkys that share your problem as do I. Mine is caused by an inability to sleep longer than 4 hrs at a time, which is a common PD complaint. Awake and doing stuff at 3:30 means that by the time that lunch is over I am pretty much exhausted so I have a nap and sleep 20 min to 1 hour and I feel completley refreshed. It is interesting however that if instead of going for the nap I opt to go for a long forced strenuous walk that I come home with a differant kind of tired and the rest of the day and the next day I feel better.
The pattern of your life does not have to fit evryone elses timetable. They will learn to get used to the fact that you may or may not be able to stay at the party and they will learn how to slice the roast themselves.
Increase coffee in a.m. Stop before 2p.m.!
About 6 weeks ago my husband's neurologist suggested Amantadine, but in a new extended release form called Osmolex (193mg) to address his fatigue concerns. Almost immediately, his energy level was significantly improved and his tremor markedly decreased. This is the first med that visibly improved his symptoms (he'd used C/L, Neuropatch, Ropinorole, et al with no visible impact). He was taking a long nap every afternoon (short naps didn't help at all) and walked around in a fog much of the day before the Osmolex. It is currently his only med beyond the thyroid meds he takes. He exercises daily and misses his boxing currently canceled for COVID distancing. I think it is about a 75-80% improvement. Stress still brings on his tremors and he sleeps longer and deeper at night than he used to, but that is acceptable. I hope this med continues to work for him.
If you eat dinner at 5 or 6, it might be low blood sugar. You could try eating some carbohydrate.
