A high B-12 level is not very common if it is not caused by supplementing. It can also be caused by kidneys that are not effectively excreting B-12, by liver issues and possibly by a specific form of leukemia.

What type of b12 had he been supplementing with? If it was cyanocobalamin I think it might not be being utilised properly. It’s synthetic and some people are unable to snip off the cyano part to use the rest. If he takes it again he should try and find a natural sourced methyl and adenosyl cobalamin which can be used more easily.
We still take methylB12. It is different than B1.
They said not take any B1 until it came down which they said tests in months
