Why Dr. C advises different oral doses (2 grams to 4 grams) for different individuals whereas the same dose of IV administration for all? Has anyone the idea
Thiamine Dosage: Why Dr. C advises... - Cure Parkinson's
Thiamine Dosage
no is different also by injection IM , for me is 3/2 at week. IM=intramuscolar
Individuals absorption through the stomach and guts varies a lot depending on the age and health condition.. The i.m. dose/blood level is much more stable.. The i.m. dose used by Dr C is not constant either..
The 100 mg intramuscular dose is his preferred method and results have been very good and reliable with that dose, so he sticks with that one, but absorption using the oral method can vary significantly per patient and can require different doses for different people. Generally he starts you at a specific oral dose based on different criteria related to PD. After initial dosing, he gets feed back from the patient to determine if he is seeing certain improvements. If he sees the improvements he is looking for, he does not change the dose, but if he doesn't see those improvements or if there is a worsening of symptoms, he will adjust the patient's dose to achieve the sought after improvements. The patients reaction to the initial dose helps Dr. Costantini to determine what dose adjustment will be needed to improve response to the thiamine protocol. It appears he has been fine tuning his approach over the past 5 years or more. Again, this is why it is advisable to stay in touch with Dr. Costantini as long as you are on his thiamine protocol, to make sure you are getting maximum benefit!

The 100 mg IM dose was too high for me. Per Dr. C I'm experimenting with oral doses now.
But I would be more prudent to make a simple account of mathematical equality between pills and injections. In my opinion, the body almost immediately expels the excess of thiamin hcl from the kidneys and sweat, so I prefer a water-soluble thiamine than a fat-soluble one.
Gio
The actual dosage with thiamine HCI could be from 500mg to 4000mg per day. Based on current accounts, Dr. Costantini often advises patients to cut down to half the initial dosage when experiencing side effects and there are many such reports here in HU. That's a big variation in dosage. The actual dosage might be variable as some patients may experience side effects even after months of use. I consider that Dr. Costantini also gains knowledge based on the feedback he receives from patients. Therefore, I suggest that patients should not assume therapeutic dosages based on other patients' accounts. Intestinal absorption is a major issue with water-soluble compounds such as thiamine HCI. Determining the ideal dosage for each patient may not be an easy task even for experienced neurologists and may involve some trial and error. Lipophilic compounds tend to have the highest intestinal absorption instead and less affected by absorption issues.
