Can anyone help me to understand the results of flowcytometry?

I live abroad and the doctor that ordered this test didn't tell me much, except that it confirms I have CLL.

How severe is it? I don't have a clue from this laconic summary.

Thanks for your help.

Hi and welcome to the group,

this result confirms your CLL at a very low level of 25% of your absolute count the time in November 2019.

CLL cell counts naturally bounce around and, especially when they are at a low level such as yours, it can look like dramatic changes but it nothing to be concerned about.

The other tests excluded a T cell malignancy and you are most likely IGHV mutated based on the negative CD38. A negative CD49b means a BTKi will be an effective treatment as a positive result for this test leads to greater adherence to CLL cells to the lymph node microenvironment. Kappa restriction result confirms the clonality of the lymphs and the smIgM tells us that there is no surface membrane IgM on the CLL cells.

This immunophenotyping is not the same as FISH testing which would tell you about the genetics of your CLL and give more information about the possible course of your CLL.

If you haven't read it, I suggest cajunjeff 's Pinned Post which include a link to FISH and genetic testing in CLL.

Thank you so much!

So you says 25% is very low level... what this percentage indicates?

Here some more details from my last blood test in April 2022 :

Leukocytes 13.7 10^9/L

Lymphocytes counts 9.00 10^9/L

Red Cells 4.4 10^12/L

Hemoglobin 13.4 g/dL

Hematocrit 40 %

Platelet 235 10^9/L

MCV 9.25 FemtoLiter

MCH 30 Picogram

In 2018 my ALC was around 13 10^9/L, so apparently it is slowly decreasing.

In this post I explain more in details my situation:

Your report says 25% of the count you had then which was approx 15, then that would be just 3.75 which is very low level. You may be on the borderline between monoclonal B lymphocytosis category (MBL) and CLL with clonal B cells around that level. The other lymphocytes that make up the count will be normal polyclonal B cells and T cells.

Fingers crossed for you that it stays low and you remain well.
