Dental Graph for dental implant

I have CLL since 2010 currently on imbrutinib. I have to have a tooth pulled and wondering if possible dental bone graph for a dental implant would be ok for someone with CLL. Any help would be greatly appreciated.

Dave, we all have such different types of cll in different stages such that it is hard to answer general questions like this. What is good for you might not be good for me. I would say for any medical procedure its a good idea to consult with your cll doctor.

That said, I had dental implants while on ibrutinib with no issues. I did not even pause my ibrutinib during the process, however I have seen where some cll doctors choose to pause ibrutinib during any procedure where bleeding is a risk.

I am very pleased, by the way, that I went the implant route. I had a couple implants that have worked our great for me thus far.

Hi Dave, I have CLL, am on low dose Ibrutinib , my last implant was horrific - bone pain - it is in my lower front jaw - no bone graph - oral surgeon almost had to remove it due to excruciating pain - I have 3 other implants in same area that are okay - after almost a year finally had crown placed - it is still “iffy.”. I told oral surgeon I will not have another. Best of luck. MsChief

Yes - I was off Ibrutinib for a week prior - I believe my immune system cannot tolerate more implants.🥴

Check with ur hematologist. Some go off of meds before dental work. Good luck! 💕

I held ibrutinib 3 days prior and one day post bone graft four months ago. No issues, still waiting for implant.

Yes, my MD said hold for three days prior, and if no bleeding start the day after. He was not a fan of me having an implant. He heard of bad results from people without disease. So was concerned about me with CLL and Ibrutinib. I have had multiple implants without any problems. Although, this is my first while in treatment.

Pre-CLL diagnosis I had a dental implant. Essentially I had it for 4 years a d it had an infection the entire time it was in. It was removed a few months after my CLL dx, while on watch and wait. It was the fourth dentist (and also not the oral surgeon nor the prosthodontist) who discovered there was a big chunk of metal missing from the implant. Called it a mechanical failure and said he had never seen anything like it. Said it had to come out because it would never seat with the prosthesis.

My heme-onc was consulted prior to removing the bad implant.

By the time we were looking at putting another implant in, I was nearing treatment. My heme-onc thought it better if I were going to do another implant there to do it before treatment began. However, I ended up not getting anything replaced there as the area had lost a lot of bone due to peri implantitis.

As others have mentioned, there are so many different CLL circumstances, it’s best to discuss / coordinate with your specialist, dentist and oral surgeon. I know the dentist always wants up to date labwork when I see him.

Hi. I have cll and have been on Ibrutinib treatment for 3 yrs. During my treatment I had to have a tooth pulled, bone graph and dental implant once the bone graph took.I had to go off the Ibrutinib 3 days prior to having my tooth pulled and bone graph. Then went back on for a couple months, then off again for the implant.

Thank you Ken but did you check with your Doctor first to find out if it was ok to do bone graph under CLL ?

I did. You beed to stop taking the Ibrutinib prior to any surgical procedure, as it has blood thinner capabilities. As your oral surgeon will most likely discuss with you, there are no guarantees that a bone graph will take. They are obligated to tell all patients that information.
