Are these early symptoms of cll?

Hello guys, I am glad to find you and through your experiences to be more informed about cll. For a year now I have constantly higher than 4500 lymphocytes and less than 2000 neutro and I ve done the the TCR PCR a year ago to check for clonality after a overpopulation of granular Tcells were found on my Facs analysis. The TCR pcr came back negative which means that the cells don’t originate from the same clone. At that point, a year ago, I was relieved and did not pursue it further. The problem is that now I have symptoms that could be early indications of a cll or indolent t-cell Leukaemia which are low grade fever around 99 with now apparent reason, sometimes swollen and tender to touch nodes under my jaw, base of throat and waistline (can it be a lymph node there? Not sure) and I feel extreme fatigue with no reason, dizziness and inner tremor all the time. I am 35 male,with normal BMI and no family history in leukemia’s except for a second aunt but not in direct relatives whatsoever. Anyone who would like to share some thoughts on all these and how to proceed would be greatly appreciated knowing of course that everything here is just opinions and my doctor would be the most appropriate to guide me through. I want this coronavirus period to pass and then go see a doctor about all these again. Not sure if you aggree with me, I would love to hear from you! Thank you in advance! Alex

Hi Alex,

Your symptoms are more likely due a low grade infection or inflammation, rather than a haemotological malignancy in my non-medically trained opinion. As you appreciate, the only way to resolve your concern is to seek medical advice from your doctor. Increasingly that's becoming possible without a physical appointment. Check if that can be done via a teleheath (i.e. phone or perhaps video) consult. If your doctor considers a blood test is appropriate, that could probably be emailed or faxed to you or your test lab. I suspect your doctor would be interested in your C Reactive Protein and Erythrocyte Sedimentation Rate tests before checking for clonal lymphocytes.

I second what Neil has said. It is likely to be something other than a blood disorder. The C Reactive Protein test would identify any infection and hence explain your enlarged lymph nodes. Once that has been established, your doctor can then begin to hone in on the nature of the problem and determine what therapeutic action to take.

If CLL is suspected a Flow Cytometry test would be next to confirm the diagnosis, but I agree with Neil and Seelel about what you might want to ask about first.

Yes I ve done a FACS and there is an abnormal high TLgL 20% which show dimmer cd5/cd7 . After pcr for clonality it came back negative. Though in the test results it is said that in 10% of neoplasias such a test is not definitely excluding a malignancy.. thus all my concerns!
