What can i give to help to make level 5 foods

I need a diet plan .Can anyone advice?

I need a level 5 food diet plan for someone with dia betes / kidney failure stage 5.and heart disease.and swallowing. Problems.No salt or sugar and no protessmen. Thank you all

Good morning Pamela, your post has really got me thinking. The dietary requirements of someone with diabetes plus the other health problems should perhaps be managed by a GP who could recommend a nutritionist to help. I hope there is help out there or maybe someone else has any ideas.
