Hi rarity depending on you and your mums circumstances you could try social services or something like Guild care if they are in your area.
Failing that you can find carers online in your area or via yellow pages.

Rarity -- Welcome. It's good that you recognize that you can't care for your mam by yourself, and have made arrangements accordingly.
Hi rarity, Welcome to this community. I have to agree with our lovely forumites, your mam should really have had a social service assessment before she was discharged from hospital. So do get in touch with them asap, as well as her GP, to arrange a district nurse visit. There will be care agencies in your mam's locality that you should be able to contact, but I would start with the above first. Here's a couple of websites for more information that may be of use too:
