Jason phone up age concern they will come and do it for you at home xxx

Jason2001, if you ring up the DWP they can sometimes do it on the phone and post it out to you for signature. I would sak Age Uk myself or your local Citizens Advice to help you. I think some of these offices have the forms. Good Luck
