Hella all. I was diagnosed a few years ago with nafld and then due to an error had a two year gap between a fibrosis score and getting the results.

That showed a jump from 7 to 16 in the score so I was offered another test. The new test gave a result of 28 and a fat scores of 396/400. Understandably I’m really quite worried and was told by the nurse it wasn’t good news.

My first question is what happens next in everyone’s experience ? Is it as bad as it looks ?

A fibrosis score of F4 ( 14 kPa or higher) means late-stage scarring or cirrhosis, where the scarring is permanent and the damage is irreversible. Gonna want to ask your doctor about this immediately.

Thanks. I’m trying to get an appointment but it’s not that easy right now. The consultant said he’d diarise a check once my results were in so hopefully I’ll hear from him soon. I’m also trying to get a feeling of the fat score (the CAP score) of 396? I feel kind of stupid as this has been going on for a few years and I never took it seriously and now I almost feel like I’ve left it too late. I have kids and the thought of telling them is killing me. We due to go on a family holiday to Florida and I want to wait until after that. The holiday is in May.

As long as you're feeling fine I wouldn't worry about it too much until anything is confirmed by GP and they run more tests. If you feel OK it may not be as bad as the nurse thought it seemed (usually that level of Cirrhosis has many obvious signs and you feel quite poor health wise). Best of luck :)

thanks for the reply. I’ll add a more detailed reply but since my last post I’ve spoken to the consultant who has confirmed the diagnosis verbally as cirrhosis and will treat me accordingly. I do have symptoms of fatigue but put it down to work stress along with the reason for generally not feeling well. Difficult to see when you’re in the middle of it. I’m waiting for the written diagnosis but am already scheduled for an endoscopy early next month. Already stopped drinking even thought it’s nafld and have started on diet changes and more exercise. Hopefully it’s not too late and don’t mind admitting this is very scary right now.

just got the results of some blood tests with an ALT of 136 and GGT of 90. Hba1c of 70 but everything else is normal. There was no ASL or is it ALS ?

The GP put in the notes the results are abnormal but expected and no further action.

Although the consultant said I’d be under their care I feel like I’m doing most of the current work in hunting down diets etc. should I be getting more help from my GP?

just got the AST result today after requesting it and it came back at 77.

Ran the numbers and they came back at a Fib4 score of 1.4 indicating mild fibrosis. This contradicts the Fibroscan result of F4 and the diagnosis of the consultant of cirrhosis. Now I’m confused !

I have an ultrasound on Monday but is this good news or is it a false hope ?

hi Brett my situation is very similar, I was told about 4 years ago that I had hepatic steotosis , nothing to worry about, fatty liver and sent me away, two years later was told the same thing, it was only when I saw a dr a year after that she confirmed the scan actually said severe fatty liver, I was shocked , they sent me for a fibroscan a month ago, my cap was 360 and my kpa was 21.5…… I knew what the numbers meant and started to cry, the tech man just said he was sorry it’s cirrotic…. I went home devastated and have not heard a thing since, no one has contacted me, I have tried twice to contact them and was told someone would be in touch, but the stress of waiting is awful….. how are you feeling now, I’m glad you have at least got follow ups etc, I just feel like I have been forgotten, I hope you have more answers soon, good luck xx

I'd chase the consultant which is what I ended up doing after a week - he called me back and explained the diagnosis and then a few days later I got a call to arrange and endoscopy which is happening today. I've already upped the exercise and changed my diet quite a bit but it's very worrying.

I have a type 2 diabetic checkup next week so will use that to ask if there is any help from the doctors.

So I guess my only advice is to start calling people and don't let them fob you off. If you have a letter from the scan call that number. I actually spoke to the secretary for the consultant and she was lovely and said she'd leave a note on his desk and he called me back a few hours later.

I had the endoscopy yesterday which showed no varices which was a huge relief. Onwards to the next step.
