Can a smooth looking liver which is normal size on an US and MRI still be cirrhotic if the damage is inside?

I still feel extremely fatigued all the time, when I used to be full of beans!

Muscle twitches, big dark circles under my eyes, dry mouth, dry veiny eyes, short term memory issues, stumbling over my words.

I’ve also been diagnosed with GS which is why my BR was 70 at one point, but my ALT and GGT were also elevated over a few months.

Ultrasound would comment on dense echo texture if cirrhosis was present & a cirrhotic liver is far from smooth - imagine a healthy liver as a smooth, juicy grape, a cirrhotic liver more like a shrivelled grape i.e. a raisin or sultana. An MRI scan basically photographs your body in thin slices so looks inside the liver not just the exterior surface.

My God snap, I have some of those exact symptoms, muscle twitching for months, dry mouth all day, dry eyes too, etc. So heres what I was told as my US and MRI and CT and bloods were all normal, have you access to a Fibrascan? They are not invasive and on my research are more accurate. Or a special form of US measuring elascity.. Heres something: Muscle twitching, Dark circles, dry mouth etc are all very much symptoms of anxiety too, have you fallen down a rabbit hole of worry?

I’ve got a fibroscan booked in on Thursday. Those symptoms could be stress/anxiety but I feel it’s more than that. Especially the fatigue. I have no energy at all. I’m actually hungry quite a lot which goes against liver symptoms. I just feel like everything is pointing to liver issues

5.4 kpa! The nurse said I looked worried. She wasn’t meant to give me the results, but she did in the end!

CAP score of 247 (shown on your screen shot of fibroscan) reveals Steatosis Grade 1 - so some fatty liver which you can't ignore and still need to work on.

About Your CAP Score

Your CAP score is a measurement of fatty change in your liver.

Your healthcare provider will use your CAP score to find out your steatosis grade.

The CAP score is measured in decibels per meter (dB/m). It ranges from 100 to 400 dB/m.

The table below shows ranges of CAP scores and the matching steatosis grade and amount of fatty change.

CAP Score Steatosis Grade Amount of Liver with Fatty Change

238 to 260 dB/m S1 11% to 33%

260 to 290 dB/m S2 34% to 66%

Higher than 290 dB/m S3 67% or more

Yes you’re right Katie. I was that fixed on the kpa I didn’t even look into the CAP score if I’m honest, so thanks for pointing that out.
