Did put comment. Data will not be accurate anyway, as you have limited people's options and are asking them to complete in a way where they may not be able to. Where is the satisfactory option. Under the MRS code of conduct you should always have a right to refuse a question, therefore a DK option should also be used. Worked in MR for many years as my husband does and was a consultant. Sorry but this survey is poorly constructed.
