blood pressure lower than usual

hi I posted a few weeks ago about my husband’s diagnosis of DCM.

After two months, his entresto dose has been increased to 49/51 twice a day.

bisoprolol remains at 1.25mg daily and empagliflozin at 10 mg daily.

I am happy to say that he is feeling much more positive and doesn’t have any symptoms at all except for the intermittent skipped heart beat.

I am wondering if to be concerned about his blood pressure. Before diagnosis and medication it was borderline high. Now it is 95/65 which is low. His consultant said it would fall due to the medication but wondering if this should be a concern and if anyone has had any similar experiences. Thanks

my suggestion would be to look at the site of Blood Pressure UK and maybe give the a ring in the morning - otherwise your doc.

hi. I too have DCM. I take the same dose of Entresto twice a day. 1.25 bisoprolol twice a day. Dapagliflozin 10mg and spiranolactone 10mg.

My blood pressure sits around 100/60 and everyone including myself are very happy with this. Initially it made me feel light headed and a few other things as well but I am now used to it and back working a very physical job. I hope this helps.

My husband is on the lowest dose of Entresto unfortunately he can't go to the next dose due to low BP, his BP is regularly in the 90's-80's over 50's- 40's. As he has no symptoms.of dizziness etc his medical team are happy with the situation as they want to treat him aggressively with the Entresto.

However I know that some people have a higher BP and are symptomatic. As everyone is different I would contact your husbands medical team and ask their advice, 95/65 is on the low side but is still within what is considered 'normal' range.

My husbands bp is similar to your husbands, he has DCM from a heart attack and is on similar meds. It has been consistently around 90/60 over the past two years. He does get light headed when he’s unwell or overdoing it physically but it doesn’t affect him on a daily basis. There would be cause for concern if your husband started to experience fainting etc but it sounds like it’s the new normal blood pressure for him.

I have dcm 10mg bisop, 49/51 entresto, 10 Dapa, 40 furosemide, 25 spiro, 10 apixiban and 40 atorvastatin and my bp is about the same. Sometimes it goes up a bit and sometimes it goes down a bit. X

Hi Lizzie, I’ve had low blood pressure for years and had syncope episodes. Was diagnosed with DCM end of 2022. Fast forward - heart pumping rate wasn’t good so now have a device fitted to regulate heart pumping rate. It’s gone up to lower normal rate now. I’m on Dapagliflozen Entresto & Bisoprolol low doses. Can’t have higher doses because of low BP. Was told to rest when dizzy look for signs if out walking like lightheaded and not to have hot baths unless I pull the plug out and cool down before I get out of the bath and same for swimming pools if doing exercise like Aquafit to sit on the side when getting out for a while until body temperature goes down because if I don’t I pass out or near faint. Was told to lie down straight away with preferably feet higher so that blood goes back up to the brain if it happens. Two months ago before I knew these things I got out of the bath and was sick and felt really ill my husband phoned for an ambulance and they took me in hospital for low BP. Blood tests and ECG’s were normal so sent home 5 hours later. This is just what happened to me so it might not happen to others it’s only my experience. You’ve just got to be careful and think about certain things you do like leave the bathroom door ajar if you take a bath or hot shower and preferably have someone in the house with you so you can shout them if you feel ill. My cardiologist said there is nothing else he can do about low BP as I’m on the lowest heart tablet doses as the tablets lower BP even more. Hope it doesn’t happen. X

Hi I have DCM and heart failure. I am roughly on the same meds has you. I could only tolerate 1 entresto 2 times a day.

All of a sudden mine is 85/61 ish. It use to be perfect for me at 105/70.

Because of persistent Afib I was on 10mg bisoprolol for 5 years, which was then reduced to 5mg recently because of a sudden drop in blood pressure, but my heart couldn’t cope so I was put on 7.5mg.

Feeling my heart racing has stopped and the breathlessness at rest, but the blood pressure is still low.

Blood pressure UK says anything under 90/60 is hypotension.

Mine runs in the 90/50s most of the time from my medications, but I am not on entresto. My Dr isnt concerned and if I feel crappy I will try to hydrate more and eat something a little salty and that tends to help. Dr said if feel dizzy all the time then to let him know so he can maybe change meds. From what I have read in this group, lots of people have a hard time with their BP when upping their entresto but it normaly will even out after a short while on it. Take care
