What should blood pressure be while on Entresto?

I am currently on the middle dose twice daily. At first, almost a year ago, my bp was 95-100/60ish. It is now 105-120/60-65. At times during the day it dips below 100. My cardiologist at Cedars Sinai in California said it needs to be closer to 100 but my 2nd opinion Cardiologist at another teaching hospital said the goal is 130/80. Can I ask thoughts regarding bp? a good source of information is the website of Blood Pressure UK, a British charity.

I believe entresto can lower BP. I think if you are okay with a lower BP then that is better for the heart than it being higher. Its quite individual thing. When I first started entresto it was very low 80/50 type stuff with 35-40bpm. My heart was badly damaged. This has gradually gone up and after 3 years it seems to have settled around 100/60-110/70. 60-75bpm. That's on middle dose. I can imagine the cardiologist may increase my dose next time I see him.

Hi am1875, are you taking entresto for heart failure? It’s my understanding that it can go to around 90 systolic but as long as your asymptomatic and your kidneys and electrolytes are ok, it’s a very good drug for HF. Check with your HF nurse.

'morning am1875, I take Entresto 24/26mg twice daily and my BP is usually 97/60 with bpm mid 80's. Take care.

Mine sometimes gets below 100, especially right after I’ve eaten. Thanks.

I’m on Entresto for heart failure and my cardiologist would agree with the first cardiologist you saw 100/70 ish. My cardiologist said they would like to keep mine as low as possible.

Because mine can dip under 100/70 I can only take 1 twice a day whereas they would prefer if I took 2 twice a day which when I did take that I kept feeling dizzy.

Hi. Yesterday for some reason it went up to 135ish/75. Don’t have any reason why but after an hour it went back down to 110ish/65. Today mostly 105-110 which may be where it’ll stay. I’d like to get it below 100 because it makes sense that the lower the bp and hr, the better as long as like someone said in an earlier post. Depends on if your kidneys are not affected, which they shouldn’t be, and you aren’t dizzy too often. Thanks.

135/75 is still good maybe you was stressed thinking about it.

Yesterday I went to my hospital appointment and mine was 100/66 that is low, the cardiologist asked if I felt faint and I said I do that’s why I use a stick to steady myself. So my next 6 monthly appointment he could do over the phone if nothing changes, or I could go in, it’s up to me as the slot is mine anyway. I thought he was being very thoughtful.

My kidney function is low, he said the meds cause that, but the meds are keeping me alive so far so they have to weigh it up and keep an eye on it.

Mine is back down in the 100-110 range. That works for me. Sometimes kidneys are compromised with all the meds but there’s more they can do as it relates to the kidneys I’ve heard. Take care.
