Steps won't be enough. You will also need to raise your heart rate significantly and keep it there for maybe at least 30 minutes. I think its swimming, running or cycling. For me cycling is the best.

Just to flag that cardio fitness is based on the amount of oxygen your body can use when exercising. To measure this properly you would need to be tested with a mask on and measuring equipment. Apple Watch monitors your heart rate while exercising and walking and uses algorithms to estimate your VO2 max. If you have a heart condition or medication that alters your heart rate it can impact the results and make them a bit unreliable. For example, mine was pretty consistent in the higher end of “average”. Immediately after my HA I was “above average” as the beta blockers slow my rate and make me appear fitter. (There is a way to record meds in app to take them into account, but seems a bit crude). I think you should take the relative reading (below, average, above) with a pinch of salt, but instead focus on your reading compared to your own previous readings. Also, don’t be disheartened (no pun intended) if you don’t see results quickly. It can take quite a bit of sustained effort to shift this metric to a meaningful degree. Definitely worth doing though (if your doctor agrees). Best wishes.

I swim or gym every day. Doesn’t seem to make any difference to my cardio level on the apple app. Still similar to yours

I recommend to anyone who exercises to read the book The Haywire Heart by Dr John Mandrola who is an electrophysiologist.

I burst out 6klms walking most days and my VO2max is generally high. I got a viral infection just before Christmas and it knocked me for six. I could not exercise very well for a few weeks and my levels dropped down a fair bit but I am now back on track. I do not pay the any real mind other than a rough guide as to the amount of exercise i am doing and the intensity. The apple watch is really good but for VO2 max its a very rough estimate.
