Have a look at MedicAlert ... though I don't have personal experience with them. There are certainly UK suppliers: one of them has leaflets in my local pharmacy.
Lots online Amazon sell them I went direct to a medical bracelet supplier online and got one with what I considered the most important of all my health problems for allergic to penicillin. It wasn't too expensive either under £12. Hope that helps.
