Two months a go I was put inAE by ambulance with AF kept in over night but not admitted Sent home with no paper work but a prescription for beta blockers and anti clotting meds I assumed that my visit would be sent to my doctorThe other day I visited the doc to sort out some more tablets as they had nt been put on repeat Was told I needed to see a doc but couldn’t so a call was made for to day When I mentioned the new tablets I was asked what I was taking them for Turns out my condition isn’t even on there system We got it sorted now and it’s been put there now and I can have my tablets Wonder whose at fault there Hospital or doctors

I’m sorry to hear that your recent diagnosis and meds were missed off your medical reports.
From personal experience, if you are not admitted, then discharge papers are not given. It sounds like the hospital forgot to write to your GP and update them on your visit to A&E. Needless to say, it has been quite a big error on their part.
Have you thought about contacting PALS (patient advice liaison service) at your local hospital to put a complaint in or discuss the situation further with them?

This is worrying reading all these things getting missed and I think it is a good reminder we need to check up ourselves as much as we can that are records are up to date even though we should not have to
I am glad yours got spotted and now updated and as to who's fault it is they no doubt will blame each other but I would have thought the Hospital would have sent this through to your Surgery but the only way to find out is phoning them to ask and if they say yes then your surgery has slipped up somewhere

Yes my experience with A&E is no paperwork but I believe an electronic transfer of info to GP. I’ve seen on my online GP records through NHS app.
In terms of fault could be either:
1. Hospital didn’t inform GP through whatever system they have.
2. GP received but didn’t action / update their system in a prompt enough manner.
3. Hospital did do its bit and GP none the wiser because IT fell down!
Lots can go wrong and unfortunately I’ve now learned to check as had a missing bit of info that made a difference to a referral.
Do you have access to your GP records online so you can check for yourself?
