I have an ICD, or as my son calls it, a pacemaker with jump leads. If my heart rhythm goes completely haywire it's capable of delivering a shock to try and get it into rhythm again. It's a mini defibrillator. I'm not really familiar with a pacemaker, as that was never an option for me. I'm sure someone here can give you a more detailed account of the difference.
There is a lot of useful information on the BHF website which I found very helpful.
