my dad had to have one when we went to Devon in July so he could have a delivery of o2 there. I cant remember if his gp surgery did the form or if dads o2 compnay did it.
Home Oxygen Order Form (HOOF). They are not just for holidays. If you're on oxygen you already have one or they would not be delivering it to you. They give you a new one if you need any changes (some need a new HOOF to get extra cylinders), if you need it to be delivered at work or if you go on holiday.

I am staying with my daughters at the moment and have a portable concentrator which was delivered to my home on Friday and I just rang airliquide and asked for it........ no HOOF

It may have soemthing to do with where you live - if you live in the south west the oxygen contact is a bit different to the rest of the UK.

I do live in the south west,and I have not had any problems with airliquide which other people on here have had.
To HOOF or not to HOOF that is the question. I had a visit from one of the bosses at Air Liquide this morning and he advised you no longer need a HOOF for holiday requests. However I say this with caution - please call and get this clarified for your particular request.
That's right you don't need a HOOF for holiday requests as long as you use the same equipment on holiday as you do at home. If you want different or more you will need a new HOOF.
Thanks for answering this question. I live in Scotland. Am on oxygen 17 hours a day but had never heard of a hoof!
