What is it please?
I have looked at it in the past and need to revisit - can’t recall why I haven’t used it though.

Is there a bronchiectasis passport?
I have some feedback: re the one-time pneumonia vaccination, I do not believe it is accurate to call it a one-off, or once in your lifetime vaccination. Maybe this description is from when ppl were not expected to last that long!
I had the vaccination in the mid-noughties, then was hospitalised with double pneumonia once in 2015 and twice in 2018. Finally I got a second pneumonia vaccination after the second bout in 2018. I have been fine since then.
Having read here people saying they had another one after around 10 - 15 years, it would be good if you would draw attention to the fact that the efficacy of these vaccinations tend to wear off at around 10 years and they can request another one (booster, top-up, whatever it is called!) Thank you.
Hi thank you for bringing my attention to this. I never knew a COPD passport existed. I have just used it and found it very useful.
I used this Patient Passport because I was told I had Emphysema by GP, given just Ventolin then ignored for 2 years. I used the report to go back (online) to Surgery although it took 4 months for a Practice Nurse to contact me and get referred to Consultant. I would recommend using this.
