Due to a number of medics.getting things wrong, I have put together a Medical Summary of my stuff to give to them in the hopes of better communication. I have included :
Name, address, phone numbers, next of kin and hospital numbers
All ailments, medication and dosage
Names and locations of consultants
I give these to all medics I see. No doubt this impresses the hell out of them!!

This is such a good idea I am definitely doing this been thinking about doing this for ages last time went to A&E they didn't have a clue how to help me x
Not being funny but some medics and doctors need this every time you go 😅. I've lost count seeing the same doctor most of the time for them to say 'still smoking'. I gave up 12 years ago. I got so annoyed last time I practically shouted 'will you please put it on my records and highlight it'. 🤣🤣🤣 . Good idea👍
I too have done this. It is unbelievable in this day and age of computers, that they can have such out of date information. When I was extremely sick in hospital at the beginning of the year, I found out they had my mum as my next of kin. She had been dead for nearly 12 years. They would have had no means of contacting any of my family (I live alone). I went back into the same hospital 2 days later and they still had the wrong next of kin on my notes! If they can get that so wrong, what else can they have wrong? Oh, yes! The fact that I am A RETAINER! 😱
I urge you all to write these things down and have them with you, thank you Shonkie for raising this, I kinda thought it was just me!

I have this written down as I am asked everytime what meds I am on. There is an nhs portal Mymft that has been set up and is really quite good for medications , appointments, blood tests , letters etc, but as usual it isnt kept properly. I spent ages filling this in with all my medications, yet when I go to an appointment they never look at it and ask my what I am taking so its a good idea to have it written down x
Hi Shonkie, you are so right. I was told to do this by my GP when attending Consultants. They were always pleased that all my details were before them instead of them having to go through my files which by this time are pretty thick. So carry on doing it. What a good idea, I have had to sort out so many errors, I find they don’t read the notes and on one occasion I was mixed up with someone else. Also my NHS medical records were lost when I moved from one area to another, I now only have medical records for the last 20 years. All lung history dating back to 1986 lost. I only found out last year when trying to retrieve hospital letters from 1986 to rectify a current issue. It has all now been completely rediagnosed, talk about reinventing the wheel.
Oh good for you.. for many years I was wrongly diagnosed, nothing to do with my now copd… I was right & all the docs were wrong.. Also, long time ago now I want to write the story of what happened to me, probably the worst ever & I was so lucky to have survived.. have the records, can’t find them atm but I will.. even went through the Tunnel so I knew how close I was.. keep going Shonkie xx
I think that's a good idea it shows that you're on the ball and I think it will make them think twice before they try and treat you or anyone as stupid I m going to do that myself
