Hi all, I've not been on for a while.. I have received my PIPS form, the only thing I haven't got is any paperwork as 'evidence' of my condition, will my GP or respiratory nurse be able to print them off my records?.. tia Baz

Yes they can and ask them for a support letter and any other health professionals you have contact with. Also call the BLF help line they can advise you how to go about filling in this form. Remember its not your illness its how it affects you they are interested in always fill in the form thinking you on your worst days because thats when we realy need the help i hope this has been of some help good luck
On the form you will be asked who is providing care. In my wife's case it was her GP, Rheumatologist, physiotherapist Also she was under the hospital pain management team. Getting evidence takes time remember that you can send your form in and send further evidence in at at later date. Just remember to put you name national insurance number at the top.
After my visitv to the consultants ... they send a letter to my GPand a copy to me detailing my health condition and how they plan to treat it ..... they do ask for your permission to consult your gp if needed and want details of hospitals and consultants...Have you received any such copy letters ? If so they should hold proof enough .
Hi poppyshola, unfortunately I haven't got any letters/plans.. all I do have at the minute are my discharge notes, rang the docs to see if they could print me something off from all my breathing tests/appointments etc.. all I got was one page 'summery'.. just repeating my meds over & over!.. rang my respiratory nurse, maybe she can help?...
