What are people doing regarding informing patients how to ""self-manage"" using action plans?

Action plansHi Heather, much depends on the patient really. Altho written AAP are recommended for all patients some don't want more paperwork so I tend to have an in-depth discussion about asthma itself, triggers, recognising worsening symptoms and what to do. Others have written plans based on peak flow measurements/symptoms but I do give every patient the Asthma UK pocket sized card re emergency treatment. We all know that most patients don't read information that is given to them and those that do are probably the ones that are more responsible about health anyway.
