whistling noise when coughing

my ds is 4 1/2 hes asthmatic and is on singulair, salbutmaol and pulmicort, hes had a cough for a few days now, which sounds very phelmy, but today when he has been coughing he makes a high pitch noise when he coughs. hes not really needed his salbutmaol much more than normal and seems ""ok"" with it.

is it normal?

It sound like he is wheezing. He has probably got an infection (may be viral). Typically we are advice to increase the dose of some of the inhalers when we see the first signs of cold (before the coughing starts) but you HAVE TO talk to your doctor first.

You say your son is on Pulmicort - Can you confirm this is via the turbohaler?

You also say that he is using his salbutamol 'more than normal'. If he is needing it on a daily basis then I suspect that his asthma is not under controll and the culprit is the turbohaler. Turbohalers require a minimum inspiratory flow rate of 60l/min and a child of 4 1/2 will be unable to achieve this. This in turn means that there is underlying inflammation is his lungs which will be causing the phlegm and wheeze. I suggest his device is changed and he has a regular review with the asthma nurse.

sorry it has taken me so long to reply thank you both

i ended up taking him to the gp on tuesday who said hes chest was clear so just give the salbutamol every 4 hours.

he has not improved at all, this morning just for walking for 10 mins he was struggling to breathe, gave him his inhaler and it worked thank god, hes cough is very fluidy and night time is bad, but also going out really sets him off.

re the pulmicort he has a inhaler which goes into a aerochamber, hes been on it since august, use to be on becotide.

not sure what else to do, he doesnt see the respiratoy speclist til april and our gp is pretty rubbish, dont really like to deal with him. hes local paed i cant get hold of as her sec is off sick so im kind of stuck

mummyto3, go back and see the doc if your son is like that, or see if you can get an appointment with the specialist soon. if the doc's as rubbish as you say, then change, it's worth asking around friends to see if they can recommend a good one.
