Afraid not, but just googled and found this on the Mind website which might help?

mind.org.uk/information-sup....

If I were to have another baby I’d definitely want to write my own care plan initially including key info like nearest relative(s).

I was slightly alarmed very recently when discussing what happened to me with PP, that a distant relative was on the verge of going to the doctors on my behalf... without speaking to my family first. Little did they know what steps had already been taken by my family to help me. I think it possibly was thought of with “the best of intentions”. But I think it’s important to say that whilst we all were / have been ill. A lot of us still have capacity at the time and should be involved in our own care wherever is feasibly possible. That’s why I believe in care plans, so that everyone is abreast of your wishes, including which people (family/friends) you want involved and those that you don’t!

Sorry to mention on this thread, but I thought it might be helpful to mention my thoughts on care plans x
