Can you help with a survey of athletes who have experienced a deep vein thrombosis (DVT), a pulmonary embolism (PE), or both (DVT and PE)?

Dr. Claire Hull at Swansea University in Wales, the United Kingdom has asked us to help find athletes who have experienced venous thrombosis. Dr. Hull has published two papers on this issue and the survey is expanding her earlier research. The survey consists of 10 easy to answer questions.
