Rivaroxaban and PPI

To those of you taking Rivaroxaban, have you all been advised to take a PPI as well , such as Omeprazole, Lansoprazole?

Maybe I should add , even though you have not been suffering from reflux or similar condition.

Yes I take 15ml lansoprazole and my husband takes 30ml Lansoprazole. All ok so far.

Thank you. Can i ask whether you had been suffering from reflux etc before being prescribed these or were these just prescribed hand in hand ?

I took lansoprazole years before Rivaroxaban but my reflux has been a bit worse since taking the anti coagulant. Hubby had Rivaroxaban when he was admitted to hospital last month and couldn’t take his usual ppi with it so was put In lansoprazole. He seems ok at the moment.

I resist any attempt to get me to take any of the azoles because once started they are almost impossible to stop and actually perpetuate a problem. At most I have taken them for two weeks only. There is so much out there about these drugs.

Totally agree - as when I had my first stroke in 2015, I was automatically given Omeprazole, Lansoprazole without real consultation with me, and lately found that long term addiction becomes a habit!

I know your feelings about PPI s Bob!

My OH has permanent asymptomatic AF and was prescribed Rivaroxaban alongside Omepraxole to ' protect his stomach'. He has never had any gastric problems and still doesn't.

He also has Parkinsons and I have been reading that these PPIs are not good for this condition so I'm advising him (nay , telling him ....) to stop taking them !! I was wondering whether PPIs are automatically prescribed alongside Rivaroxaban

Laziness on the part of doctors in my view or maybe they get paid to prescribe them? Either way its usually a one way trip. Took six months of hell to get off them before but thank goodness I learned what I did.

I was prescribed an azole when first diagnosed. No history of reflux etc. I remember reading some advice from you Bob, and looking into it. I haven't taken it since (4years) and all OK on Rivaroxaban. Thank you!

Not here is South Africa, I was prescribed Xarelto and the only other recommendation with it was Q10 enzyme. I have been taking it for 5 years with no problem - have recently asked my doctor to change me to Eliquis after reading reports here that it is the better drug for A Fib, and have been on it for a month with no problem and no suggestion that I take anything extra with it.

Thank you. That is the drug , known in the UK as Apixaban, that I'm requesting my husband be changed to.

I was on Rivaroxaban for about 3 years. I was prescribed PPI’s but couldn’t tolerate them. The Rivaroxaban caused gastritis and a gastric ulcer with recent bleeding.

I refused to take Rivaroxaban after that. I now take Dabigatran which, touch wood, is fine.

Yes I did, though that was never on the instructions. I never take any anticoagulants on an empty stomach. I certainly would never chance Rivaroxaban again.

I was sent a message from the Pharmacist to start taking Omiprazole with my Riveroxaban, after reading up the pros and cons I refused. Plus I don't suffer from reflux.

Wife suffers from bad reflux and it ruined her QoL. It appears to be hereditary as both her mum and our oldest boy suffer from it also. GP refused to put her on Omeprazole a number of times. But as soon as she was diagnosed with PAF and put on Rivaroxaban she was prescribed Omeprazole. In her view its the one silver lining of her PAF diagnosis.

Thank you. I'm very similar in that I really need to take PPI, in my case Lansoprazole as I have a hiatus hernia ( incredibly risky to operate on according to gastroenterlogist...). I have no problems with it.

In my husbands case I believe this is possibly contributing to an escalation of his symptoms of parkinsons, namely depletion of dopamine reserves.

No I've never had it with my Rivaroxiban, been on that for 2 years now. never had reflux with it either. My partner has Omeprazole with naproxin for gout though.

If your doctor has any reason to suspect there's any risk of intestinal bleeding (which I gather is a relatively common thing, especially in people who have ever taken drugs such as aspirin, ibuprofen or naproxen), then lansoprazole or a similar PPI has now been shown to protect the intestinal surfaces well.

I take esomeprazole and rivaroxaban, but I do have bad reflux problems.

Yes - my wife has to take one if she takes naproxen for her back troubles.

Despite the often bad press the drugs received in this particular forum, I have read that they’ve been shown to be unusually safe even in the longer term and a gastroenterologist I saw was very much in favour of them. As a prophylactic, though, I’m less sure.

hi Jalia I was on omoprozole before my heart issues. They immediately changed it to lanzoprozole. I asked about it and the consultant they routinely give something to protect the stomach with all the heart medications. And theoretically lanzoprozole is safer with heart medications than omoprozole but that’s only theoretically.

I have been taking rivaroxaban for a number of years. I wasn't prescribed P.P.I. although l used to take ibuprofen before l was on an anticoagulant. I don't get indigestion or acid reflux , so l am not going to take an unnecessary pill. I didn't even know that patients are prescribed them with anticoagulants. My doctors have never mentioned them.

No, unfortunately, it was banned. My GP prescribed omeprazole as re placement but I didn’t take it because of effects of lansaprazole when I was prescribed it after ablation (diarrhoea).

I've been on Rivaroxaban (Xarelto) for years and was never advised to take a PPI, and don't need it.

No. advised to think about regular lansaprozole but didn't want to mess up my body balance. As BobD commented didn't want to perpetuate another problem cause by this type of meds
