Best notify insurance company. It'll raise the premium for sure. Brothers went up for angina. Still waiting on my insurance to even contact me when I had to cancel holiday because of afib first week October.
Apparently especially high if you're going to the U.S...
We've used All Clear and Flexicover (brokers). Did on line. Make sure everything is declared. Haven't been to the States for many years.Check who the underwriters of any policies are, some all the same, but vast difference in price.

My late husband and I could only use All Clear due to his complicated cancer journey. The only claim that we ever had was for me spending a night in the ships hospital due to AF. I had declared it and the claim was paid in full.
The vast majority of insurance companies will cover AF for an additional premium.
Your brother must declare his AF and of course any other medical conditions that he may have.
Try Staysure, been with them for years and one of the best for covering medical conditions. When I was diagnosed with AF my policy was hardly affected.
