Warfarin protocol

Not so long ago, someone put up the protocol for warfarin levels, with particular relevance to inr readings that were either too high or too low, and the recommended corrective dose. Would they be kind enough to repost please? Thank you.

It would be quite wrong for anybody here to advise on INR and warfarin dose. We are all far to different and metabolise the drug at different levels for there to be a one size fits all formula. One needs history over time and even then as we all know the computer generated doses used by many med centres are hopeless in a lot of cases anyway. Sorry. If only there was such a thing!

I dont think I made my post terribly clear, it was an official paper from the relevant authorities giving the information, which was given!

Annaelizabeth I think you are referring to my post- it is a protocol that was online and I had asked my GP if it was the relevant one for me- I think it's best to follow Bob's advice above- if you search for protocols online - you can then take them to your GP and ask if they would be relevant for you??
