Converting from Warfarin to Rivaroxaban

My GP said stop Warfarin for two days then start the Rivaroxaban. My last INR was 1.7 and from what I have read it seems you should start Rivaroxaban when INR is 2-3. Any thoughts?

You can start it cold so why would you need to be in range if you never took warfarin before. There is no testing of range with the NOACs so it should be irrelevant

Thanks Bob, but I have been on Warfarin for a year now but found that I am allergic to it! My last INR was today with a reading of 1.7. Thus the reason to try the next one!

I am on Rivaroxaban with no pretesting! Hope it helps!

Look online clear guidelines for converting from warfarin to newer anticoagulants. I changed to Pradaxa INR had to be below 2 before I could start new drug. Stopped warfarin for few days to drop to 1.9 good luck
