Requesting Experiences and Opinions on 177Lu-TLX591.
I was in this trial 2 years ago and had a pretty bad experience ending up in hospital for 2 weeks with thrombocytopenia. I had blood clots and couldn’t pee. It was pretty painful with the bladder filling and a lot of pumping. I think it’s a large dose of the Lu177 that’s given and this caused my platelets to drop very low to 15. The Trials Oncologist commented a few ended up in hospital but none for as long as 2 weeks. It did drop my PSA from 169 to 59 after 30 weeks until progression. Good luck, and watch your platelets and red blood cells.

I actually start chemo tomorrow. I had been on Abiraterone after coming off of the Lu177 J591 trial and responded well bringing my PSA down from 150 to 24 over almost 12mths. It’s now up to 60.
I don’t know the amount of the dosage as I was never told nor was it in the notes, but I had 2 lots of treatment 3 weeks apart and told to keep 6ft away from anyone and sleep in separate bedrooms for 2 weeks after the 2nd dose.
Another member, Mateo Beach, did the treatment also and had very little side effects.
I had one round of treatment (split into 2 doses) of 177Lu-TLX591 (formerly known as J591) in Perth by Dr. Nat Lenzo, per his trial protocol but I was not in the trial as am mHSPC. This followed SBRT of my two identified LN mets (oligometastatic). This monoclonal antibody radioligand has much higher binding characteristics for PSMA than does Pluvicto. The intent was to possibly clear micrometastases.
Side effects were some diarrhea for one week. And some drop in blood cell counts, which were mild for me and recovered within 3 months. But it can be severe and require transfusions in some.
The results for me has been excellent. my PSA dropped by 90% initially then became undetectable (< 0.015) by 6 months. It remains undetectable now going on almost 3 years. MB
