My dad’s (diagnosed with high volume metastatic PCa in March and immediately began triplet therapy) most recent PSMA/PET scan seems mostly positive except for the first finding
“1. Suspicious for local recurrence at the prostate left apex peripheral zone”
He will have a new PSA test on Monday before our MO appointment next Wednesday, but his number last month was .04.
If PSA remains steady are we most likely looking at a “wait and see” approach or should I expect perhaps a medication change? This is our first experience with possible recurrence and I am just trying to prepare myself for what could come next

Unfortunately the triple therapy is very effective against cancer but it will not destroy it completely. There will be a recurrence at some time. A PSA value of 0.04 is no recurrence yet. I would try to get the prostate radiated now in addition to the hormone therapy hoping that this will extend the time to the recurrence. Other patients will just continue with the combination therapy while it works.
Wanted to follow up and let you know we had the oncology appointment today and it seems we’re headed in the direction you suggested. Setting up an appointment with a radiologic oncologist next. PSA did hold steady at .04 this month
