Feel this virtual “malecare” space has become my family and go to place for every question big or small I had when we started our journey when my husband was diagnosed this May. And will continue to be a space I will keep coming back to for advice, solace and encouragement. Wanted to thank you all for all your support, advise, expertise and humor through this difficult time. He rang the bell today and we celebrated with our kids with a chick fillet breakfast-his favorite! 🙂

Awesome, and here's to a long term wonderful outcome for y'all

My 28 IMRTs at AIS in 2023 so far has worked, the 39 chemos from 2015 til 2023 slowed the PCa down a bit. So far no SEs from the IMRTS and I made my 10 yr Cancerversary when PSA was 552.2 on 10/13/2014, now at 0.6

You don't mention ADT, so be sure he takes it after the radiation, which I am sure MSK will ensure. I rang the bell after prostate radiation, but it remained there and spread to a node. I rang the bell after pelvic radiation, but it spread to a higher node. ADT from the beginning might have prevented the spread.

ADT in my opinion is a pause button just delaying things if salvage treatment has failed. It's not curative. I believe it's supposed to make the cancer more sensitive to radiotherapy so many believed it improved the chances of the salvage RT being curative. But the RADICAL HD Trial indicated otherwise:

The RADICALS-HD trial (results first published in 2022 I believe) did not show a significant improvement in overall survival when adding a short course of ADT to post-operative radiation therapy for prostate cancer.

While it did demonstrate a benefit in terms of improving metastasis-free survival with a longer course of ADT, the impact on overall survival was not significant.

Of course it delayed MFS. Because you pressed the pause button for a while. How many more additional months or years of improved QOL do you get in return? I have no clue. Hopefully it's at least as long as the duration of time you had to endure the lower QOL due to the ADT side effects.
